Submit your application by March 16th.
How do I submit an application?
- There are 2 ways to apply as a Citizen Member:
- Fill out the online application form (link) before March 16th, 2025, at 11:59 p.m.
- Print, fill out, and mail the application form no later than March 16th, 2025, to the following address:Subject: CSBE Consultation ForumTango – HR Solutions394 boulevard Maloney Ouest, Suite 106Gatineau, Quebec, J8P 6W2
- The Health and Welfare Commissioner does not receive any applications directly. In accordance with its Regulation, the CSBE has entrusted the process of selecting Citizen Member applications to Tango – HR Solutions, an independent and objective organization. They will respond to requests for information, receive your applications and pre-select them for the 2025–2028 edition of the Forum.
What should I include in my application?
To apply for the Forum, you must include one of the following documents with your application form:
- A text (between 50 and 350 words)
- A video or audio recording (maximum 2 minutes – only the first 2 minutes will be evaluated).
The document should describe your background, interests, motivations and what interpersonal skills you feel are necessary to participate optimally in Forum sessions.
The application form also includes the following:
- A section where you must provide your personal contact information
- Eligibility and exclusion criteria
- Questions about your sociodemographic profile (age, gender, identification with an ethnocultural minority group or as a member of the First Nations, Inuit or Métis communities, etc.)
- Questions about your experience and résumé (training, civic engagement, etc.)
- Questions about accessibility
Your answers to the sociodemographic questions will help us select a membership that’s as diverse as possible to represent Quebec’s population. They will not be used to exclude or select candidates on such grounds.
Your experience and résumé will be used to select members with diverse experiences.

Eligibility criteria and selection procedure
Tango – HR Solutions will evaluate the applications. Here are the steps your application may go through.
Step 1. Verifying the application’s eligibility (February 10th to March 16th, 2025)
What are the selection criteria?
- Be a Canadian citizen or have permanent residence status in Canada
- Be 18 years of age or older on May 1, 2025
- Reside in the health region indicated on the application form
- Be available, apart from exceptional circumstances, for 3 Forum sessions per year over 3 years (October 2025 to June 2028).
What are the exclusion criteria?
The following individuals are not eligible:
- Employees or board members of the following entities:
- Ministère de la Santé et des Services sociaux
- Santé Québec
- A health and social services agency, including the Nunavik Regional Board of Health and Social Services
- The Cree Board of Health and Social Services of James Bay
- A health and social services institution or any other organization providing services related to the health and social services sector and receiving a grant from an agency or the minister
- A professional order in the health and social services sector
- Régie de l’assurance maladie du Québec or an entity receiving remuneration from the latter or that has entered into a service contract under section 259.2 of the Act respecting health services and social services
- Registered in the registry of lobbyists under the Lobbying Transparency and Ethics Act.
Step 2. Assessing the required skills and qualities (ongoing from February 10th to April 11th, 2025)
If your application is eligible, Tango – HR Solutions will assess the following skills and qualities:
- Your motivation and interest regarding issues related to the health and social services system, as well as population health and welfare.
- Your commitment to civic engagement.
- Your skills in terms of communication, collective reflection, information sharing and deliberation.
- Your interpersonal skills, such as your ability to work on a team, communicate effectively, listen, be open to diverse perspectives and new ideas, and to demonstrate good judgment, tact and discretion with respect to the confidential content of the deliberations.
Tango – HR Solutions will then select a diverse pool of candidates. As much as possible, the Forum’s members should reflect the characteristics of Quebec’s population. Therefore, Tango – HR Solutions will also consider that criterion.
Step 3. Participating in a mock online deliberation session (April 2025)
You may be asked to participate in a mock deliberation session as a final selection step. The sessions will require some preparation time and will be held in April 2025. You will be offered various time slots, as needed, to accommodate you. The goal of this step is to assess your skills in terms of communication, collective reflection, information sharing and deliberation.
Step 4. Submitting applications to the Commissioner for final selection (June 2025)
In May 2025, Tango – HR Solutions will submit at least 3 applications per region to the Commissioner. In June 2025, the Commissioner will then select 1 application per region to make up the Consultation Forum.
Tango – HR Solutions will inform the individuals whose application was not successful. The remaining applications submitted to the Commissioner will be placed on a list of substitutes who can participate if a candidate withdraws.
