Call for applications to the Consultation Forum of the Health and Welfare Commissioner

Information regarding the application process 

The application form will be active for the duration of the call for applications, from February 10th, 2025, to March 16th, 2025, at 4:59 p.m. If you have any questions during this period, please contact Tango – HR Solutions:

Please also note that to complete your application form, you will need to provide the following: 

  • An explanation of your background, interests and reasons for applying; and 
  • An explanation of the interpersonal skills that you feel are necessary to participate optimally in Forum sessions.  

More information will be provided in section 5. 

 

These explanations may be provided as a text (50 to 350 words) or as a video or an audio recording up to 2 minutes long. 

Lastly, depending on how the process unfolds and the applications received, you may be invited to participate in one of the mock deliberation sessions scheduled for the final selection step. This will require some preparation time, and the sessions will be held in April. Various time slots will be offered, if necessary, to accommodate participants. The goal of this step is to assess your skills in terms of communication, collective reflection, information sharing and deliberation. 

 

Privacy and protection of personal information 

Tango – HR Solutions has been mandated by the Health and Welfare Commissioner (CSBE) to carry out the call for applications and will be collecting the information necessary to assess your application. Only authorized personnel of Tango – HR Solutions will have access to the information you submit through this form and during subsequent steps of the selection process, if applicable. 

Measures have been taken to ensure the protection of personal information submitted using this form (see the CSBE’s Privacy Sment [in French], which sets out the conditions for the protection of personal information collected electronically).  

As stated on the site’s previous information pages (Make Yourself Heard), the information you provide is required to assess your application based on the selection criteria determined by the CSBE and the eligibility and exclusion criteria set out in the Act respecting the Health and Welfare Commissioner and its Regulation. That information will be used by the firm Tango – HR Solutions for the sole purpose of compiling your application file. The information will only be sent to the CSBE if your application is selected to be on the list of applications proposed to the CSBE for its Consultation Forum. Your information will be kept by the firm for the duration of the Forum members’ term, so that other applications can be suggested to the CSBE in the event the list of applicants is depleted. After the Forum term, the information will be securely destroyed. 

The information necessary to assess your application will be collected through this form (and the documents you provide as attachments, if applicable) and through your participation in a group interview (a mock deliberation session), if your application is chosen for that second step.  

Your participation in this call for applications is entirely voluntary. However, if you would like to apply to become a member of the Consultation Forum, it is mandatory to provide all information requested, and an incomplete form could result in your application being rejected. Note that at any time, even after submitting your form, you can terminate the application process by contacting the firm using the contact information provided at the beginning of the form. Your application will then be withdrawn, and any information you have already submitted will be destroyed. 

Finally, at any time, you can exercise your right of access and rectification in relation to the information you have provided. To do so, contact (specify the name of the resource person at Tango). 

 

Instructions for filling out the form 

Creating a user account is recommended so that you can return to the form at a later time. 

To create a user account, you will need to enter a valid email address.  

You will receive a message at that email address confirming that your account has been created.  

Your answers will be automatically saved when you move from one page to the next within the form. You can edit a given response by clicking the “Back” button. 

You can stop entering your answers and come back to the form later, as long as you have not yet submitted it. Once you have submitted your form, it will no longer be possible to make changes. 

You will have access to a printable version of your completed form for your records.
